Where to Order Simmons High Chair Parts

I am looking for information on where to order Simmons High Chair Parts. I would like to order two (2) brackets for a wooden Simmons high chair that is made out of ash. The manufacture date for the baby high chair is 1988 and I bought the high chair for my son in that same year.
The problem with it is that the tray will not stay on so that I can use it for my grandson. I need replacements for the brackets that are attached to the tray.

This wooden baby high chair was manufactured in Canada, but the manufacturing company is listed as Simmons, New London, WI 54961 but I can't find a way to contact them to see where to order Simmons High Chair Parts.
From what I can tell, it looks like the part number is 0532 and was made in 1988 which is the year the baby high chair was manufactured.
Could someone that stops by Unique Baby Gear Ideas please help or give me a contact number?
