I am looking for help identifying a Simmons baby crib so I can locate the correct replacement parts. Both top guide components on this crib are cracked, and replacements cannot be ordered without the original model number.

The manufacturer label was scratched off while the crib was previously in use, so the model number is no longer available on the frame.
