Need Help Identifying This Simmons Baby Crib

Simmons Baby Crib Upper Track Top Guides in Natural Color
I need help identifying this Simmons baby crib! I need to order both top guides as they both have cracks in them and I can't since I don't have the Model Number.
The sticker was scratched off the crib when it was in use by another baby.
Has anyone tried ordering Simmons crib parts from anyone other than the company without a model number? Did the parts fit and was the cost reasonable?
I'm afraid that it would be a waste of time and money to order without all of the information requested by the Simmons customer service department even if ordering from a generic parts supplier so I REALLY need help identifying this Simmons baby crib so that I will have the model number to simplify my search.
